For the second time in three years, a Guardians pitcher walked off the mound at T-Mobile Park while shaking their arm due to injury.
The first time that happened, Shane Bieber underwent season-ending and career-altering Tommy John surgery.
The recent occurrence came with ace Tanner Bibee, who left the Guardians’ win on Thursday after completing his warm-up pitches in the bottom of the sixth inning.
He was eventually diagnosed with “right shoulder inflammation,” which is an ominous diagnosis that should send chills up every Guardians’ fans spine.
And while Bibee tried to assuage those fears with his postgame quotes, it’s never a good sign when your ace leaves Game 1 of 162 with an arm injury.

Bibee spoke to reporters in Seattle after the game, where he said that he was feeling good and was feeling optimistic about the injury.
“It was kind of off and on the entire time,” Bibee said, per MLB.com’s Tim Stebbins. “It kind of got to a point where I feel like I couldn't really battle through it and give my best stuff. It got to a point where I didn't want to hurt myself or the team in turn.”
He also underwent tests after the game, which he said he felt “encouraged” by. Bibee allowed three earned runs with seven strikeouts in five innings prior to getting hurt.
Bibee made the right call by leaving the game early as opposed to trying to push it on an injured shoulder, but it’s hard to believe that this injury won’t impact the future of the Guardians’ rotation in some way.
It’s a different situation than Bieber’s, who said in an interview that he knew early in his 2024 start that he was going to have to undergo Tommy John surgery.

Meanwhile, Bibee said that his injury is something that just popped up that he didn’t know about beforehand. He had an up-and-down 2025 season but finished the year well and pitched well in the spring.
Logan Allen will likely be the next man up if Bibee has to miss any time. He was fighting with Parker Messick for the final spot in Cleveland’s rotation but was optioned to Triple-A at the end of spring training.
While he’s not on the same level as Bibee, he’s a solid reinforcement option given he has 379 innings of big league experience under his belt.
It gets a little dicey after him, however. Yorman Gómez, Doug Nikhazy, Yorman Gómez and Austin Peterson are the other starting options at Triple-A who are on the 40-man roster. Nikhazy is the only player among that bunch with any kind of MLB experience.
Kolby Allard is also at Triple-A and is coming off a season where he had a ton of success pitching in a variety of roles. That said, he’s not on the 40-man roster, so the Guardians would need to make some kind of other roster move to free up space for him.
The Guardians managed to best the Mariners on Thursday thanks to a strong showing from their offense and bullpen, but they’re going to need more from their starting rotation if they want to truly compete in 2026.
